Youth Worker (Outreach)

  • Remuneration SCHCADS level 5 dependent on qualifications and experience + 10.5 % Employer superannuation

 

About the position

The purpose of the position is to reduce the harms associated with problematic substance use in the community which seeks to engage young people 12-21 in public settings, complete assessments and provide AOD information, brief interventions and health promotion. This position is supported by the direct manager with collegial support from YSAS primary health staff and works closely with YSAS General Practitioners, community General Practitioners and Day Program staff.

Thus, integral to this role is a willingness to participate in pro-active engagement with young people in the community to promote their access to health services. This may involve some afterhours work.

 

Some of your key responsibilities will be:

  1. Complete Assessments on young people with complex clinical presentations and provision of clinical services to young people with complex problems related to their use of alcohol and drugs.
  2. Undertake outreach to initiate and maintain contact with young people at risk of disengagement from services and community.
  3. Implement a range of modalities including but not limited to: intensive individual casework, group work, family work, out postings, community outreach, collaboration and care planning.
  4. Advocate on behalf of young people in the target population and in consultation with other key stakeholders.
  5. Provision of a developmentally appropriate and responsive service to vulnerable young people from communities in the Northern region of Melbourne.
  6. Facilitate a range of responses that meet the young persons assessed needs, such as accommodation, health, food and safety, education, employment and social activities.
