About the position
Funded through SEMPHN’s GP Multidisciplinary Team (MDT) program, this role supports quality use of medicines, shared decision-making, and inclusive, trauma-informed care.
The General Practice Pharmacist plays a key role in delivering safe, evidence-based, and youth-friendly medication management within a multidisciplinary general practice team. This position is focused on improving the health outcomes of young people aged 12–25 by promoting quality use of medicines, supporting shared decision-making, and improving access to accurate and inclusive health information.
About you and how you will be successful in this role:
Working collaboratively with General Practitioners (GPs), Allied Health Practitioners, Nurses, and other team members, the pharmacist contributes to an integrated model of care that addresses the complex and diverse health needs of young people—including those from LGBTQIA+ communities, culturally and linguistically diverse backgrounds, and other priority populations.
In line with program objectives, the pharmacist will contribute to integrated team-based models that increase access to primary care, participate in digital health initiatives (e.g. MyHealth Record, POLAR, Florence), and support outcome reporting through tools such as EQ-5D-5L surveys. The role also promotes continuous improvement in medication safety, prescribing practices, and staff education across general practice settings.
