About the position
The role will provide youth-friendly, evidence-based nutrition support to young people aged 12–25 years as part of a multidisciplinary team at headspace Frankston. This role aims to address nutrition-related chronic conditions, disordered eating, and food insecurity while supporting early intervention and whole-of-person care.
This position is funded through the South Eastern Melbourne Primary Health Network (SEMPHN) as part of the General Practice Multidisciplinary Team (GP-MDT) initiative. The purpose of the GP-MDT grant is to support small and solo general practices to enhance access to integrated, team-based care for priority and under-served populations.
About you and how you will be successful in this role
The Accredited Dietitian will play a key role in providing in-reach allied health services, collaborating and care planning in consultation with GPs and other health professionals to deliver coordinated, youth-focused care and improve health outcomes in the local community.
The successful applicant must be eligible to deliver Medicare services and have demonstrated experience and skills in the evidence-based assessment and treatment of eating disorders.
